The Virginia Vital Records Office is responsible for maintaining and issuing certified copies of vital records for events that occurred in Virginia.

- APPLICATION FOR CERTIFICATION OF A BIRTH RECORD (VS6B)
- APPLICATION FOR CERTIFICATION OF A DEATH RECORD (VS6D)
- APPLICATION FOR CERTIFICATION OF A MARRIAGE AND/OR DIVORCE RECORD (VS6MD)
